El artículo desarrolla una reinterpretación de la vacunación en Colombia como un bien preferente, enmarcado en la existencia de mercados incompletos con expresión territorial diferenciada. El problema de investigación se centra en las persistentes desigualdades en la cobertura entre departamentos, especialmente en aquellos caracterizados por elevados niveles de pobreza, alta ruralidad y diversidad sociocultural. Se plantea como hipótesis que estas brechas no son aleatorias, sino resultado de la interacción estructural entre factores económicos, institucionales, geográficos y culturales que limitan la eficacia de la provisión pública. Metodológicamente, se adopta un enfoque mixto que articula análisis cualitativo estructural con estimaciones econométricas mediante mínimos cuadrados ordinarios aplicadas a los 32 departamentos, incluyendo pruebas de validación estadística. Los resultados muestran que la pobreza emerge como el determinante más significativo de la cobertura, mientras que la ruralidad y la capacidad institucional presentan efectos interdependientes condicionados por problemas de multicolinealidad. En consecuencia, se concluye que la vacunación debe entenderse como un bien preferente territorialmente condicionado, cuya provisión exige el diseño de políticas públicas diferenciadas, el fortalecimiento de las capacidades institucionales y la incorporación de enfoques interculturales que reconozcan la heterogeneidad del territorio colombiano.

This article proposes a reinterpretation of vaccination in Colombia as a merit good, integrating structural analysis and econometric evidence within the framework of territorialized incomplete markets. The research problem focuses on persistent coverage gaps across departments, particularly in regions characterized by high poverty, rurality, and cultural diversity. The hypothesis states that these inequalities result from the interaction of economic, institutional, geographic, and cultural determinants that constrain the effectiveness of public provision. Methodologically, a mixed approach is employed, combining qualitative-structural analysis with econometric estimation using ordinary least squares (OLS) across 32 departments, complemented by validation tests. Results show that poverty is the main determinant of vaccination coverage, while rurality and institutional capacity exhibit interdependent effects shaped by structural multicollinearity. The study concludes that vaccination cannot be understood as a homogeneous good but rather as a territorially conditioned merit good requiring differentiated policies, institutional strengthening, and intercultural approaches.
